4. All additional membership fees of $25 will be waived.

5. It is your responsibility to ensure there are sufficient funds available in your account to cover the weekly auto-debit. If an auto-debit is declined due
to insufficient funds the transaction will fail which requires additional administration and yields additional bank costs payable by you. Ezidebit is the third party used to process transactions and is unable to absorb these costs and applies an $11.90 fee for these failed transactions. This fee is debited 7 days after the failed payment.

6. If an auto debit cannot be made for any reason Ideal Physiotherapy may in its absolute discretion effect payment after the due date upon receipt of
sufficient monies in your account.

7. After the 12 consecutive week minimum term, memberships may be suspended via a written request (via email to idealphysio@gmail.com) for a

You will not be able to book another class that week without an additional purchase. If you are unable to attend a booked class please cancel so that your place may be made available to others.

In the event of scheduled public holidays, modified clinic hours or client illness we cannot offer refunds, credits, exchanges or transfers. All classes missed due to illness will be forfeited by the client.

9. If auto-debit payments fail for 1 week Ideal Physiotherapy will cancel your membership until full payment is made.

10. The terms and conditions of our third party biller Ezidebit form a binding component of your membership agreement. We can email you these terms and conditions or you can view them via Ezidebit’s website under https://www.ezidebit.com/ /media/ezidebit/files/ezidebit- terms-and- conditions-aus.pdf
